How referrals work

You tell us how to reach you and, in general terms, what support you think is needed.
We call you to discuss the situation and whether we can help in the person's area.
We agree a secure handover of the person's details and relevant clinical information.
We contact the person or their family to arrange an assessment.

We'll keep you informed, with the person's consent, as the referral progresses.

Urgent situations

This form is not monitored around the clock. If a patient is being discharged today and needs urgent support, say so when we speak, but always follow your organisation's own safe-discharge procedures.
